QC looks fine, patients shifted

Anyone else seeing a 3–4% positive shift in ALT on a Cobas c502 after moving to lot 24–011 this week? Cal and two-level QC are within Westgard, but side-by-side patient comparatives vs last week’s lot and our backup analyzer suggest control commutability issues — how are you documenting lot-to-lot verification when controls pass but patient deltas don’t?

I’d run 20 patient splits vs backup with Deming, document ‘lot 24–011’ bias, and add a temporary LIS comment — agree?

What moved the needle for me was switching my title to ‘Sr. Scientist, Cereal/Extrusion’ and front‑loading RVA/Mixolab, NIR, and ‘Clextral Evolum 25’ in the resume — filters caught it and callbacks picked up. Small caveat: the hybrid roles often hide under QA/Ops tags like Ingredient Quality or Milling R&D, so include those in your searches.

I keep a one-liner: “we can usually hit two of three — meds, docs, or cheapest — pick your priorities.” I run their 14 meds on Find a Medicare plan, then toggle preferred pharmacies and 90‑day mail; if they won’t budge on the cardiologist, I call the plan’s pharmacy line to see if a tiering exception on the priciest drug will swing it. Occasionally GoodRx beats the plan in the deductible phase, but I warn them about coordination and tracking totals.

, seen this on c502 when controls aren’t commutable; for “lot 24–011” I’d build a 3-level patient serum pool (low/med/high), assign targets from last week’s lot, and run a short bridge per EP26, then stash the bias summary and vendor commutability note in the lot file (good primer: 404 Not Found - Westgard QC). If the ALT shift is proportional, kick it to Roche and consider holding the lot; if it’s a flat about 3–4% you can justify use with a brief LIS advisory plus tighter delta checks. Are your high-end ALTs showing a bigger delta than normals?
